Horizon Care Services provides Personal Care Assistance (PCA) and Community First Services and Supports (CFSS) to help individuals live safely and independently at home.
What We Offer
We follow Minnesota DHS guidelines to deliver high-quality, Medicaid-funded services:
Personal Care Assistance (PCA)
Support for individuals with disabilities, chronic illnesses, or age-related conditions:
- Activities of Daily Living (ADLs): Dressing, grooming, bathing, eating, mobility, transfers
- Instrumental ADLs: Meal prep, shopping, managing finances
- Health-Related Tasks: Medication assistance, range-of-motion exercises
- Behavioral Support: Observation and redirection for safety
- Service Models: Traditional PCA and PCA Choice (client-directed)
Community First Services and Supports (CFSS)
CFSS expands on PCA with more flexibility and control:
All PCA services plus:
- Budget model for managing care expenses
- Paid family caregivers (including spouses and parents of minors)
- Consultation services to help clients plan and direct care
- Financial Management Services (FMS) for payroll and taxes
- Participant-directed staffing and scheduling
CFSS Agency Model Services Horizon Care Services is a certified provider of PCA and CFSS services under Minnesota’s agency model. We: – Hire, train, and supervise support workers – Help implement the Service Delivery Plan created by your consultation provider – Support paid family caregivers, including spouses and parents of minors (starting Oct 1, 2024) – Use the CFSS training budget to tailor worker education to each client’s needs.
CFSS officially replaced PCA/CSG on Oct 1, 2024, and Horizon is fully enrolled as a CFSS agency provider.
Note: Horizon does not provide consultation services. Clients work with a separate consultation provider to develop their plan before Horizon begins service coordination.

- Must live in the community
- Enrolled in Medical Assistance
- Assessed by county or managed care organization
- Able to direct care or have a representative
